Job Description
Deliver high-quality, patient-centered care as a Home Health Registered Nurse, providing vital healthcare services directly to patients in their homes. This is a full-time, 13-week contract position designed for a self-driven RN with a passion for home-based care. Your clinical expertise and organizational strengths will be instrumental in supporting positive patient outcomes and ensuring compliance with home health standards.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Perform comprehensive OASIS assessments and implement individualized care plans based on patient needs
- Deliver skilled nursing care, such as wound care, medication administration, IV therapy, and chronic disease management
- Monitor patient status, communicate changes promptly with the care team and physicians, and adapt care plans as needed
- Educate patients and their families on treatment plans, medications, and preventive health practices
- Ensure accurate, timely documentation within Home Care Home Base (HCHB)
- Collaborate closely with physical therapists, occupational therapists, social workers, and home health aides for effective interdisciplinary care
- Participate in staff meetings, case conferences, and ongoing education initiatives
- Abide by home health policies, as well as state and federal compliance guidelines
Qualifications:
- Active RN license in good standing
- Minimum of 1 year recent home health nursing experience required (field experience preferred)
- Proficient in Home Care Home Base (HCHB) and OASIS documentation
- Strong critical thinking, assessment, and organizational skills
- Reliable transportation, valid drivers license, and appropriate auto insurance
- Ability to handle a caseload of 2532 points per week; BSN and Medicare home health knowledge preferred
