Deliver compassionate, high-quality care as a Home Health Registered Nurse serving patients in their homes. This fulfilling contract role blends strong clinical skills with the autonomy of self-directed fieldwork. Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Role Details:
- Full-time, 13-week contract
- Caseload: 2532 points per week (typically 35 patient visits per day)
- Field-based position with daily travel to patient homes (mileage reimbursement provided)
- Electronic documentation via HCHB
Primary Responsibilities:
- Complete OASIS assessments and develop individualized care plans
- Deliver skilled nursing services, such as wound care, IV therapy, medication administration, and chronic disease management
- Monitor patient status, document changes, and communicate with the care team and physicians
- Educate patients and families about health conditions, treatments, and preventive care
- Collaborate with an interdisciplinary team including therapists and home health aides
- Accurately document all care activities using Home Care Home Base
- Attend case conferences and contribute to ongoing quality improvement
Qualifications:
- Current RN license in good standing
- Minimum of 1 year of recent Home Health field experience
- Strong proficiency with HCHB and OASIS
- Excellent clinical assessment, time management, and communication abilities
- Own reliable transportation, valid drivers license, and auto insurance
- Ability to independently manage a flexible schedule
Preferred:
- BSN degree
- Prior experience managing 2532 points weekly
- Knowledge of Medicare/home health compliance standards
