Deliver high-quality, patient-centered care as a Home Health Registered Nurse. This contract role offers the opportunity to bring your clinical expertise and compassionate approach to individuals in their own homessupporting health, independence, and positive outcomes.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
- Caseload: 2532 points per week (typically 35 patient visits daily)
- Field-based: daily travel to patient homes within your assigned territory
Key Responsibilities:
- Complete comprehensive OASIS assessments and develop individualized care plans.
- Provide skilled nursing services, such as wound care, IV therapy, medication administration, and chronic disease management.
- Monitor patient progress, promptly identify changes in condition, and communicate with physicians and the healthcare team.
- Educate patients and families on disease processes, treatment plans, and medication management.
- Maintain accurate documentation in Home Care Home Base (HCHB) and adhere to compliance standards.
- Collaborate with physical therapists, occupational therapists, medical social workers, and home health aides.
- Participate in case conferences, staff meetings, and ongoing education.
Qualifications:
- Active RN License in good standing for the state of practice
- At least 1 year of recent Home Health experience (field experience preferred)
- Proficiency with Home Care Home Base (HCHB) required
- OASIS assessment experience required
- Reliable transportation, valid drivers license, and current auto insurance
- Strong clinical assessment, communication, and organizational skills
- BSN and knowledge of Medicare/home health compliance standards preferred
- Previous caseload management of 2532 points per week is a plus
