Seeking a skilled and compassionate Registered Nurse to provide home-based care for patients, ensuring safety, comfort, and high-quality outcomes. This contract opportunity offers the chance to make a direct impact, utilizing expertise in patient-centered nursing in a field-based setting.

Schedule:

• Monday to Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract
• Patient caseload: 2532 points weekly (approximately 35 visits per day)
• Daily travel to patient homes within an assigned territory (mileage reimbursement included)

Responsibilities:

• Complete thorough OASIS assessments, develop and implement personalized care plans.
• Deliver skilled nursing services: wound care, IV therapy, medication administration, and chronic disease management.
• Evaluate and document patient progress in Home Care Home Base (HCHB); detect changes and promptly communicate updates.
• Educate patients and their families on conditions, treatments, medications, and ongoing care.
• Collaborate with other health professionalsPT, OT, MSW, aidesto coordinate effective, interdisciplinary care.
• Participate in case conferences, meetings, and professional development as needed.
• Maintain compliance with all regulatory policies and standards.

Qualifications:

• Active RN license in good standing.
• At least one year of recent Home Health nursing experience (field work preferred).
• Proficient with Home Care Home Base (HCHB) and OASIS.
• Valid drivers license, reliable vehicle, and current auto insurance.
• Strong assessment, clinical judgment, and problem-solving abilities.
• Self-motivated, dependable, and able to manage an independent field schedule.
• BSN and prior experience with similar caseloads preferred.
• Knowledge of Medicare and home health standards is an advantage.
