Job Description
An experienced and compassionate Home Health Registered Nurse is needed to deliver skilled nursing care to patients in their homes. This position is ideal for an RN who thrives on patient-centered care and values autonomy, organization, and excellence in clinical practice.
Schedule/Hours:
- Monday through Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
- Caseload: 2532 points per week (approximately 35 patient visits per day)
- Field-based: Daily travel within assigned territory (mileage reimbursement provided)
Key Responsibilities:
- Perform comprehensive assessments and develop individualized care plans using OASIS tools.
- Deliver skilled nursing interventions, such as wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient progress, promptly recognizing and communicating changes in condition with the healthcare team.
- Educate patients and families on treatments, medications, and preventive strategies to foster independence and improved health outcomes.
- Document care accurately and timely in Home Care Home Base (HCHB).
- Collaborate with therapists, social workers, and aides for a multidisciplinary approach to care.
- Participate in staff meetings, ongoing education, and case conferences.
- Comply with all regulatory guidelines and agency standards.
Qualifications:
- Active and unencumbered RN license in the state
- At least 1 year of recent home health (field) experience
- Proficient with HCHB and OASIS
- Solid assessment, communication, and organizational abilities
- Reliable transportation, valid drivers license, and auto insurance
- Independence and flexibility in managing a field-based caseload
Preferred:
- BSN degree
- Previous experience with 2532 points per week
- Familiarity with Medicare and home health compliance
