An excellent opportunity awaits for an experienced and compassionate Registered Nurse specializing in home health care to make a meaningful impact on patients lives. This position focuses on delivering high-quality, patient-centered nursing care directly in patients homes, ensuring continuity, safety, and optimal health outcomes.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• 13-week contract commitment
• Caseload: 2532 points per week (typically 35 home visits each day)

Key Responsibilities:

• Complete thorough OASIS assessments and develop individualized patient care plans.
• Deliver skilled nursing care, such as wound care, IV therapy, medication administration, and chronic disease management.
• Monitor patient progress, promptly noting and reporting any changes to the wider care team.
• Provide patient and family education on care plans, disease management, and preventive strategies.
• Accurately document all care in the Home Care Home Base (HCHB) system.
• Coordinate with other disciplines including therapy, social work, and home health aides for effective, holistic care.
• Maintain compliance with all agency and government regulations and participate in team meetings and professional development.

Qualifications:

• Current, active RN license (in good standing).
• At least 1 year of recent, hands-on home health nursing experience.
• Proficient with Home Care Home Base (HCHB) documentation and OASIS assessments.
• Reliable personal transportation, valid drivers license, and auto insurance.
• Strong ability to work independently, manage a robust field-based schedule, and communicate effectively.
• BSN and prior experience with higher caseloads preferred.
• Familiarity with Medicare and home health compliance standards is a plus.

$1,845 - $1,952/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
