An exciting opportunity is available for a skilled and compassionate Registered Nurse to provide patient-centered care in the home health setting. This contract role involves ensuring high-quality nursing services for patients in their homes, focusing on both clinical excellence and strong communication.

Schedule:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM

Responsibilities:

• Perform comprehensive assessments using the OASIS tool and develop individualized care plans.
• Deliver skilled nursing interventions such as wound care, IV therapy, medication administration, and chronic disease management.
• Track patient progress, recognize changes in condition, and communicate promptly with physicians and the interdisciplinary care team.
• Educate patients and families on diagnoses, treatments, medications, and preventive measures.
• Maintain accurate, timely documentation in the Home Care Home Base (HCHB) system.
• Collaborate with physical therapists, occupational therapists, medical social workers, and home health aides to ensure coordinated care.
• Take part in case conferences, staff meetings, and educational sessions to ensure up-to-date best practices.
• Uphold all regulatory, compliance, and documentation standards.

Qualifications:

• Active RN license (in good standing) in the state of practice.
• At least one year of recent field-based home health RN experience.
• Proficiency with Home Care Home Base (HCHB) and OASIS assessment required.
• Solid clinical assessment abilities and strong critical thinking.
• Excellent communication, time management, and organizational skills.
• Reliable transportation, valid drivers license, and auto insurance required.
• BSN preferred.
• Experience managing caseloads of 2532 points per week and knowledge of Medicare/home health compliance are considered strong assets.

$2,011 - $2,127/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
