Job Description
A skilled and compassionate Home Health Registered Nurse (RN) is needed to deliver patient-centered care within a field-based role. This 13-week contract position focuses on providing expert nursing services directly to patients in their homes, ensuring quality outcomes and continuity of care.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Daily travel to patient homes within your designated territory (mileage reimbursement offered)
- Caseload: 2532 points per week (generally 35 patient visits daily)
Primary Responsibilities:
- Complete comprehensive OASIS assessments and develop individualized care plans
- Deliver skilled nursing care including wound care, IV therapy, medication administration, and chronic disease management
- Closely monitor patient progress, promptly communicating changes with the interdisciplinary team and physicians
- Educate patients and families regarding conditions, medications, treatment plans, and prevention
- Maintain up-to-date documentation using Home Care Home Base (HCHB) software
- Collaborate with physical therapists, occupational therapists, social workers, and home health aides to ensure cohesive patient care
- Attend regular case conferences, staff meetings, and participate in required continued education
- Adhere to regulatory and agency documentation and clinical standards
Qualifications:
- Current and active RN license (in good standing) in state of practice
- Minimum of one year of recent home health nursing experience (field-based experience strongly preferred)
- Proficiency using Home Care Home Base (HCHB) and experience completing OASIS assessments
- Excellent clinical assessment and critical thinking abilities
- Strong communication, organization, and time management skills
- Reliable transportation, valid drivers license, and current auto insurance
Preferred:
- Bachelor of Science in Nursing (BSN)
- Familiarity with managing a caseload of 2532 points
- Understanding of Medicare and home health compliance protocols
