An opportunity is available for an experienced and compassionate Registered Nurse to deliver skilled nursing care in a home health setting. This role ensures patient-centered care and clinical excellence, with responsibilities spanning from comprehensive assessments to interdisciplinary coordination.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Perform thorough OASIS assessments and create individualized care plans.
- Deliver skilled nursing interventions, such as wound care, medication administration, IV therapy, and management of chronic conditions.
- Monitor and evaluate patient progress, promptly reporting any changes to care teams and physicians.
- Educate patients and their families on disease processes, treatment regimens, medications, and strategies for preventive care.
- Accurately document all care in the Home Care Home Base (HCHB) system.
- Collaborate effectively with physical and occupational therapists, medical social workers, and home health aides to achieve coordinated care.
- Take part in case conferences, staff meetings, and ongoing professional development as required.
- Adhere to regulatory and agency protocols for all documentation and care delivery.
Qualifications:
- Active RN license in good standing.
- At least one year of recent home health (field-based) experience.
- Proficient in using Home Care Home Base (HCHB).
- Experienced with OASIS assessments.
- Strong assessment skills and critical thinking ability.
- Reliable transportation, valid drivers license, and proof of auto insurance.
- Ability to work autonomously and balance a flexible field schedule.
- Excellent communication, time management, and organization.
Preferred:
- BSN degree.
- Experience managing a caseload of 2532 points per week.
- Familiarity with Medicare and compliance standards in home health.
