Unique opportunities are available for an experienced and compassionate Registered Nurse specializing in home health to deliver high-quality patient care in a field-based setting. This contract role focuses on ensuring patients receive exceptional nursing attention and continuity of care in their own homes.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Role Overview:
- Full-time, 13-week contract
- Field-based visits (patient homes) within an assigned territory
- Caseload expectation: 2532 points per week (typically 35 patient visits daily)
- Use of Home Care Home Base (HCHB) for documentation and OASIS for assessment
- Daily travel required (mileage reimbursement provided)
Key Responsibilities:
- Conduct OASIS assessments and implement individualized care plans
- Deliver skilled nursing care, including wound care, medication administration, IV therapy, and chronic disease management
- Monitor and evaluate patient conditions, communicating changes to the interdisciplinary team and physicians promptly
- Educate patients and families on treatment, medications, and disease prevention
- Maintain timely, accurate electronic documentation in HCHB
- Collaborate with therapists, social workers, and aides for coordinated care
- Engage in case conferences, staff meetings, and ongoing professional development
- Ensure compliance with all regulatory and care delivery standards
Qualifications:
- Valid RN license in good standing in the state of practice
- Minimum 1 year of recent home health experience (field work highly preferred)
- Proficiency in Home Care Home Base (HCHB) and OASIS required
- Strong clinical assessment and independent decision-making skills
- Reliable transportation, valid drivers license, and auto insurance
- Excellent organizational, communication, and time management abilities
- BSN and experience managing a 2532 point caseload preferred
- Familiarity with Medicare and home health compliance standards is beneficial
