An exciting opportunity is available for an experienced and compassionate Registered Nurse specializing in home health services. This field-based contract role centers on delivering skilled nursing care to patients in their own homes, supporting their recovery and independence while upholding high clinical standards and strong patient relationships.
Schedule:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Caseload: 2532 points per week (typically 35 patient visits daily)
- Daily travel to patient residences within a designated territory (mileage reimbursement provided)
Key Responsibilities:
- Perform comprehensive OASIS-based assessments, create and update individualized care plans, and deliver skilled nursing interventions such as wound care, medication administration, IV therapy, and chronic disease management.
- Monitor and evaluate patient progress, identifying any changes in condition and ensuring swift communication with healthcare teams and physicians.
- Educate patients and families regarding disease processes, care plans, preventive strategies, and medication regimens.
- Maintain accurate, timely documentation using Home Care Home Base (HCHB).
- Collaborate with interdisciplinary teamsincluding therapists, social workers, and aidesfor cohesive patient care.
- Participate in case conferences, staff meetings, and continuing education.
- Ensure strict adherence to regulatory guidelines and agency procedures.
Qualifications:
- Active and unencumbered RN license in Mississippi
- Minimum 1 year recent Home Health RN experience (field-based preferred)
- Proficient in Home Care Home Base (HCHB) and OASIS
- Proven clinical assessment, organization, and communication skills
- Valid drivers license, reliable transportation, and auto insurance
- Ability to work independently and manage care within a flexible, mobile schedule
Preferred:
- BSN degree
- Experience managing a caseload of 2532 points per week
- Knowledge of Medicare and home health compliance
