An experienced and compassionate Home Health Registered Nurse (RN) is needed to provide skilled nursing care in patient homes within a full-time, 13-week contract position. This role requires delivering quality, patient-centered care with a focus on clinical excellence and effective communication while working independently in a field-based setting.

Location:
This opportunity is based in Minter, AL, a welcoming community known for its scenic surroundings and southern charm. Schedule/Hours:

• Monday Friday
• 8:00 AM 5:00 PM

Key Responsibilities:

• Perform comprehensive OASIS assessments and develop individualized care plans
• Deliver skilled nursing services including wound care, medication administration, IV therapy, and chronic disease management
• Monitor patient progress, identify changes in condition, and communicate promptly with healthcare teams and physicians
• Educate patients and families about disease processes, treatments, and preventive care
• Maintain accurate and timely documentation using Home Care Home Base (HCHB)
• Collaborate with interdisciplinary team members such as therapists, social workers, and aides
• Participate in case conferences, staff meetings, and continuing education
• Comply with all regulatory and agency standards for documentation and patient care

Qualifications and Desired Experience:

• Active RN license in good standing for the state of practice
• Minimum one year of recent home health nursing experience, preferably in the field
• Proficiency with Home Care Home Base (HCHB) and OASIS assessment tool
• Strong clinical assessment, critical thinking, and communication skills
• Reliable transportation with valid drivers license and auto insurance
• Ability to work independently and manage a flexible schedule

Preferred:

• BSN degree
• Experience managing 25-32 points per week caseload
• Knowledge of Medicare and home health compliance standards
