Job Description
An experienced and compassionate Home Health Registered Nurse (RN) is needed to provide skilled nursing care in patient homes within a full-time, 13-week contract position. This role requires delivering quality, patient-centered care with a focus on clinical excellence and effective communication while working independently in a field-based setting.
Location:
This opportunity is based in Minter, AL, a welcoming community known for its scenic surroundings and southern charm. The area offers a peaceful environment with easy access to nearby cities for dining, shopping, and recreation, making it an ideal place to balance professional responsibilities and personal life.
Schedule/Hours:
- Monday Friday
- 8:00 AM 5:00 PM
Key Responsibilities:
- Perform comprehensive OASIS assessments and develop individualized care plans
- Deliver skilled nursing services including wound care, medication administration, IV therapy, and chronic disease management
- Monitor patient progress, identify changes in condition, and communicate promptly with healthcare teams and physicians
- Educate patients and families about disease processes, treatments, and preventive care
- Maintain accurate and timely documentation using Home Care Home Base (HCHB)
- Collaborate with interdisciplinary team members such as therapists, social workers, and aides
- Participate in case conferences, staff meetings, and continuing education
- Comply with all regulatory and agency standards for documentation and patient care
Qualifications and Desired Experience:
- Active RN license in good standing for the state of practice
- Minimum one year of recent home health nursing experience, preferably in the field
- Proficiency with Home Care Home Base (HCHB) and OASIS assessment tool
- Strong clinical assessment, critical thinking, and communication skills
- Reliable transportation with valid drivers license and auto insurance
- Ability to work independently and manage a flexible schedule
Preferred:
- BSN degree
- Experience managing 25-32 points per week caseload
- Knowledge of Medicare and home health compliance standards
