Job Description

Deliver compassionate, expert care to patients and families by supporting individuals facing terminal illnesses in their homes or residential settings. This contract opportunity is ideal for registered nurses who thrive in patient-centered environments, possess excellent clinical judgment, and bring empathy to end-of-life care.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Occasional weekend or on-call rotation may be required

Key Responsibilities:

  • Complete comprehensive assessments and develop individualized hospice plans of care
  • Provide skilled nursing focused on symptom management, pain relief, and patient comfort
  • Administer medications, perform treatments, and implement interventions per physician orders and hospice standards
  • Monitor patient condition changes, updating the medical director and interdisciplinary team as needed
  • Educate patients, families, and caregivers on disease processes, symptom management, and emotional support
  • Work collaboratively with social workers, aides, chaplains, and volunteers to address holistic needs
  • Document visits and care in Home Care Home Base (HCHB) accurately and promptly
  • Participate in team meetings, case conferences, and continuous improvement initiatives
  • Travel daily to various patient locations within the assigned service area (mileage reimbursement provided)

Qualifications:

  • Active, unrestricted RN license in the state
  • At least 1 year recent hospice RN experience
  • Proficiency in Home Care Home Base (HCHB)
  • Strong clinical assessment, communication, and interpersonal skills
  • Knowledge of pain management, end-of-life, and family support principles
  • Reliable transportation, valid drivers license, and auto insurance
  • Self-motivated and comfortable managing a field-based, autonomous schedule
  • BSN and experience in palliative care preferred
  • Familiarity with Medicare hospice regulations and documentation preferred

$2,016 - $2,132/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
