An exciting opportunity is now available for an experienced and compassionate Registered Nurse with a strong background in home health care. In this field-based role, you will deliver high-quality nursing care to patients in their homes, playing a crucial part in ensuring continuity and patient-centered care within a supportive healthcare team.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Qualifications:
- Active RN license in good standing
- Minimum of 1 year of recent home health field experience
- Proficiency in Home Care Home Base (HCHB)
- OASIS assessment experience required
- Strong clinical assessment and critical thinking abilities
- Reliable transportation, valid drivers license, and auto insurance
- Skilled communicator with exceptional organizational and time management skills
- Ability to work independently, efficiently managing a flexible patient caseload
Preferred Skills:
- Bachelor of Science in Nursing (BSN)
- History of managing 2532 points per week
- Understanding of Medicare and home health compliance standards
Core Responsibilities:
- Perform comprehensive OASIS assessments and develop individualized care plans
- Provide skilled nursing interventions, including wound management, medication administration, IV therapy, and chronic disease education
- Monitor patient progress, promptly identifying and reporting status changes
- Educate patients and families about diagnoses, treatments, and preventive care
- Maintain accurate, timely documentation in HCHB
- Collaborate with physical/occupational therapists, social workers, and home health aides for coordinated care delivery
- Attend case conferences, staff meetings, and pursue ongoing education
- Travel daily within your assigned territory (mileage reimbursement provided)
