Deliver high-quality care and make a meaningful impact as a Home Health Registered Nurse (RN). This contract opportunity offers dedicated nurses the chance to support patients in their homes, ensuring comprehensive, compassionate, and patient-centered healthcare. You will operate independently, using your clinical knowledge to assess, educate, and oversee the care journey for each individual.
Schedule & Role Details:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Field-based: daily travel to patient homes (mileage reimbursement provided)
- Caseload: 2532 points per week (35 patient visits per day)
- Utilize Home Care Home Base (HCHB) and OASIS for documentation and assessment
Responsibilities:
- Conduct comprehensive OASIS assessments and develop individualized care plans
- Deliver skilled nursing care: wound care, medication administration, IV therapy, chronic disease management
- Monitor and evaluate patient progress, communicating changes promptly to the healthcare team and physicians
- Educate patients and families on care plans, disease processes, and preventive strategies
- Maintain up-to-date and accurate documentation in HCHB
- Collaborate with therapy staff, social workers, and home health aides for coordinated support
- Participate in case conferences, team meetings, and continuing education
- Uphold all regulatory standards for home health care delivery
Desired Qualifications:
- Active RN license (state of practice, in good standing)
- Minimum 1 year of recent Home Health experience (field experience strongly preferred)
- Proficient in HCHB and OASIS
- Strong clinical assessment, time management, and organizational skills
- Reliable vehicle, drivers license, and insurance required
- BSN preferred; experience managing 2532 points per week and knowledge of Medicare/home health standards desirable
