An experienced Home Health Registered Nurse is needed to deliver skilled nursing services to patients in their homes under a full-time, 13-week contract. If youre highly organized, self-motivated, and enjoy working independently, youll thrive in this field-based position interacting directly with patients and their families.

Schedule/Hours:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract
• Caseload: 2532 points per week (typically 35 patient visits per day)
• Daily travel within an assigned territory (mileage reimbursement provided)

Key Responsibilities:

• Perform in-depth OASIS assessments and build individualized care plans
• Deliver skilled nursing care, including wound care, IV therapy, chronic disease management, and medication administration
• Monitor patient progress and communicate changes to the wider care team and physicians
• Educate patients and families on disease processes, care plans, and preventive health
• Complete accurate, timely documentation using Home Care Home Base (HCHB)
• Collaborate with interdisciplinary teams (therapists, social workers, aides) for cohesive patient care
• Participate in case conferences, ongoing education, and staff meetings
• Ensure compliance with agency, state, and federal standards

Qualifications:

• Active, unencumbered RN license
• Minimum 1 year of recent field-based Home Health RN experience
• Proficient in Home Care Home Base (HCHB) and experienced with OASIS assessments
• Excellent clinical assessment and time management skills
• Reliable transportation, valid drivers license, and auto insurance
• Strong communication and organizational abilities
• BSN and prior experience managing a 2532-point caseload preferred
• Knowledge of Medicare and home health compliance standards beneficial
