We are seeking a skilled and compassionate Home Health Registered Nurse (RN) for a full-time, 13-week contract opportunity. Youll serve as a crucial care provider, delivering comprehensive, patient-centered nursing services within a supportive, autonomous environment.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Caseload: 2532 points per week, averaging 35 patient visits daily
- Territory-based daily travel (mileage reimbursement provided)
Key Responsibilities:
- Perform thorough OASIS assessments and develop individualized plans of care
- Deliver skilled nursing interventions such as wound care, medication administration, IV therapy, and chronic disease management
- Monitor patient status, communicate significant changes with the interdisciplinary care team, and implement updates as needed
- Educate patients and families on disease processes, medications, and health maintenance
- Document care promptly and accurately using Home Care Home Base (HCHB)
- Coordinate with therapists, social workers, and home health aides to provide comprehensive services
- Attend staff meetings, case conferences, and ongoing education sessions to maintain best practices
- Ensure compliance with regulatory guidelines and standards of care
Qualifications:
- Active RN license (state of practice), in good standing
- Minimum 1 year of recent home health nursing experience
- Proficiency in Home Care Home Base (HCHB) and OASIS documentation required
- Strong assessment, time management, and organizational abilities
- Reliable transportation, valid drivers license, and current automobile insurance
- BSN preferred; experience managing 2532 points/week and Medicare compliance knowledge a plus
