Job Description

Deliver high-quality nursing care where it matters mostin patients homes. As a Home Health Registered Nurse, youll serve as the critical link between patients, families, and their multidisciplinary care teams, ensuring that each individual receives comprehensive, patient-centered support. This full-time, 13-week contract position is ideal for professionals who value autonomy, strong clinical standards, and a dynamic, field-based work environment.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Caseload: 2532 points weekly (typically 35 patient visits per day)
  • Field-based, direct travel to patient homes (mileage reimbursement provided)

Key Responsibilities:

  • Conduct thorough OASIS assessments and develop personalized care plans.
  • Deliver skilled nursing interventions, including wound care, IV therapy, medication administration, and chronic disease management.
  • Closely monitor patient progress, promptly recognize changes in condition, and communicate effectively with physicians and interdisciplinary team members.
  • Educate patients and families on care procedures, disease management, medication regimens, and preventive strategies.
  • Accurately document all assessments and care interventions in Home Care Home Base (HCHB).
  • Participate in interdisciplinary case conferences and ongoing professional development.
  • Ensure full compliance with all regulatory guidelines and agency documentation standards.

Qualifications:

  • Active RN license in the state (in good standing)
  • At least one year of recent home health nursing experience (field-based preferred)
  • Proficient in Home Care Home Base (HCHB) and OASIS assessments
  • Excellent organizational, communication, and time management skills
  • Reliable transportation, valid drivers license, and auto insurance
  • BSN and experience managing 2532 caseload points weekly preferred
  • Knowledge of Medicare and home health care regulations is a plus

$1,970 - $2,084/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
