Job Description

Delivering skilled care to patients in a home-based setting, this position is well-suited for a Registered Nurse with a professional commitment to clinical quality and patient-centered outcomes. Schedule:

  • Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

  • Conduct OASIS assessments and create individualized plans of care to address patient needs
  • Deliver skilled nursing interventions such as wound care, medication administration, IV therapy, and chronic condition management
  • Monitor patient progress and respond promptly to any changes in condition by coordinating with other care team members and physicians
  • Educate patients and families regarding treatment plans, medication regimens, disease processes, and preventive strategies
  • Enter timely documentation into the Home Care Home Base (HCHB) electronic charting system
  • Collaborate with therapists, social workers, and home health aides to provide well-coordinated and comprehensive care
  • Attend case conferences, staff meetings, and participate in ongoing professional development

Minimum Requirements:

  • Active RN license in good standing for the state of practice
  • Minimum 1 year of recent home health nursing experience (field-based strongly preferred)
  • Proficient in Home Care Home Base (HCHB) and experienced with OASIS assessment tools
  • Strong clinical assessment, organizational, and multitasking abilities
  • Reliable transportation, valid drivers license, and proof of auto insurance
  • Ability to manage a field-based schedule and caseload (typically 2532 points per week)

Preferred Qualifications:

  • Bachelors degree in Nursing (BSN)
  • Experience with Medicare/home health compliance
  • Prior caseload management matching 2532 points/week
