Job Description
Deliver high-quality, patient-centered care as an experienced Home Health Registered Nurse. This field-based opportunity allows you to make a meaningful difference by bringing skilled nursing services directly to patients in the comfort of their homes.
Schedule:
- MondayFriday, 8:00 AM 5:00 PM
Your role will include:
- Conducting thorough OASIS assessments to guide and create individualized care plans
- Delivering skilled nursing interventions such as wound care, IV therapy, medication administration, and chronic disease management
- Monitoring patient progress, identifying changes in condition, and keeping care teams and physicians promptly informed
- Providing hands-on education for patients and families regarding diagnoses, medication management, and preventive measures
- Utilizing Home Care Home Base (HCHB) for timely and precise documentation
- Collaborating with a multidisciplinary teamincluding therapists and home health aidesto ensure coordinated, holistic care
- Participating in case conferences, staff meetings, and ongoing education opportunities
- Maintaining full compliance with regulatory requirements and agency policies
What were looking for:
- Active and unencumbered RN license in the state of practice
- At least 1 year of recent, field-based home health RN experience
- Demonstrated proficiency in Home Care Home Base (HCHB) and OASIS
- Strong clinical assessment, problem-solving, and organizational abilities
- Reliable transportation, valid drivers license, and current auto insurance
- Ability to thrive independently in a flexible, mobile work environment
- Excellent communication and time-management skills
Preferred:
- BSN degree
- Experience managing a 2532 point weekly patient caseload
- Familiarity with Medicare and home health compliance
