Delivering skilled care to patients in a home-based setting, this position is well-suited for a Registered Nurse with a professional commitment to clinical quality and patient-centered outcomes. Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Conduct OASIS assessments and create individualized plans of care to address patient needs
- Deliver skilled nursing interventions such as wound care, medication administration, IV therapy, and chronic condition management
- Monitor patient progress and respond promptly to any changes in condition by coordinating with other care team members and physicians
- Educate patients and families regarding treatment plans, medication regimens, disease processes, and preventive strategies
- Enter timely documentation into the Home Care Home Base (HCHB) electronic charting system
- Collaborate with therapists, social workers, and home health aides to provide well-coordinated and comprehensive care
- Attend case conferences, staff meetings, and participate in ongoing professional development
Minimum Requirements:
- Active RN license in good standing for the state of practice
- Minimum 1 year of recent home health nursing experience (field-based strongly preferred)
- Proficient in Home Care Home Base (HCHB) and experienced with OASIS assessment tools
- Strong clinical assessment, organizational, and multitasking abilities
- Reliable transportation, valid drivers license, and proof of auto insurance
- Ability to manage a field-based schedule and caseload (typically 2532 points per week)
Preferred Qualifications:
- Bachelors degree in Nursing (BSN)
- Experience with Medicare/home health compliance
- Prior caseload management matching 2532 points/week
