A fulfilling opportunity awaits experienced Home Health Registered Nurses (RN) seeking to make a difference through compassionate, high-quality care delivered directly in patients homes. This contract position is ideal for professionals who value autonomy, clinical excellence, and meaningful patient relationships in a field-based setting.
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
- Caseload: 2532 points per week (typically 35 visits daily)
Qualifications:
- Active RN license in good standing
- Minimum of one years recent home health experience (field experience preferred)
- Proficiency in Home Care Home Base (HCHB) electronic documentation
- OASIS assessment experience
- Excellent assessment and critical thinking skills
- Reliable transportation, valid drivers license, and auto insurance
- Strong communication, time management, and organizational skills
- BSN and experience managing 2532 points per week preferred
- Understanding of Medicare and compliance standards a plus
Responsibilities:
- Complete comprehensive OASIS assessments; develop and manage individualized care plans
- Deliver skilled nursing care such as wound care, medication administration, IV therapy, and chronic disease management
- Monitor patient progress, respond to changes in condition, and communicate proactively with physicians and the interdisciplinary healthcare team
- Educate patients and their families on treatments, medications, and ongoing care
- Accurately document all patient care and activities in HCHB
- Coordinate closely with therapists, social workers, and home health aides to ensure seamless patient care
- Engage in case conferences, staff meetings, and ongoing education sessions
