An exciting opportunity is available for an experienced and compassionate Registered Nurse specializing in Home Health. This role centers on delivering skilled nursing care to patients in their residences, ensuring continuity, clinical excellence, and patient-focused outcomes. Ideal candidates are self-directed professionals who excel at working autonomously, are detail-oriented, and demonstrate strong clinical assessment abilities.
Schedule/Hours:
- Monday Friday, 8:00 AM 5:00 PM
Key Qualifications:
- Active RN license in good standing (state of practice)
- At least 1 year of recent home health nursing experience, preferably field-based
- Demonstrated proficiency with Home Care Home Base (HCHB) electronic documentation
- Experience conducting OASIS assessments
- Reliable transportation, valid drivers license, and auto insurance
- Excellent communication, clinical, and organizational skills
- Ability to operate independently, manage a dynamic home-based caseload, and maintain high standards of documentation
- BSN preferred
- Familiarity with Medicare and compliance standards a plus
Role Responsibilities:
- Complete thorough OASIS assessments and develop individualized care plans for each patient
- Deliver skilled nursing interventions including wound care, IV therapies, medication administration, and chronic disease management
- Monitor patient progress and quickly identify any changes in condition, facilitating prompt action and communication with the care team
- Educate patients and caregivers on disease management, care plans, and prevention strategies
- Maintain timely and accurate electronic records using HCHB
- Foster collaboration with other healthcare professionals, including therapists, social workers, and aides
- Participate in staff meetings, continuing education, and case conferences as needed
- Uphold regulatory compliance and best practices within all aspects of care delivery
