Bring your expertise as a Home Health Registered Nurse (RN) to a fulfilling contract opportunity providing compassionate, individualized care to patients in their homes. This role emphasizes skilled nursing, patient advocacy, and clinical excellence within a supportive, field-based setting.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Contract Details:

• Full-time, 13-week duration
• Field-based: Daily travel to patient homes within an assigned territory (mileage reimbursement included)
• Caseload: 2532 points per week (typically 35 patient visits daily)
• Charting: Home Care Home Base (HCHB) required
• OASIS assessment tool utilized

Responsibilities:

• Conduct OASIS assessments and craft comprehensive, individualized care plans
• Deliver skilled nursing services, including medication administration, wound care, IV therapy, and chronic disease management
• Monitor and document patient progress, quickly responding to changes in condition and collaborating with physicians and the healthcare team
• Educate patients and families on disease processes, medications, and care strategies
• Accurately maintain electronic nursing documentation via HCHB
• Coordinate care with therapists, social workers, and home health aides
• Participate in staff meetings and case conferences, maintaining compliance with home health standards

Qualifications:

• Active RN license in good standing for state of practice
• At least 1 year of recent Home Health (field) experience
• Proficiency with HCHB electronic documentation and OASIS required
• Strong assessment, communication, and organizational abilities
• Reliable transportation, valid drivers license, and current auto insurance
• Ability to work independently and manage a flexible schedule
• BSN and prior experience with 2532 point caseloads preferred
• Familiarity with Medicare and home health regulations

$1,970 - $2,084/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
