In this dynamic role, youll exercise autonomy while collaborating closely with interdisciplinary teams and ensuring high standards of clinical excellence. This contract position is field-based and supports a busy weekday schedule.

Schedule/Hours:

• Monday Friday, 8:00 AM 5:00 PM

Role Highlights:

• Conduct comprehensive OASIS assessments and develop individualized patient care plans.
• Deliver skilled nursing care such as wound care, IV therapy, medication administration, and chronic disease management.
• Collaborate with physical and occupational therapists, medical social workers, and home health aides to ensure coordinated, patient-centered care.
• Monitor patient progress, quickly communicate any changes to the care team, and adapt care plans as needed.
• Educate patients and their families on diagnoses, medications, and self-care.
• Utilize Home Care Home Base (HCHB) for timely, accurate electronic documentation.
• Daily travel to patient residences within an assigned territory (mileage reimbursement included).
• Maintain compliance with all regulatory, clinical, and documentation standards.
• Participate in case conferences, staff meetings, and ongoing education.

Qualifications:

• Active RN license in good standing in Pennsylvania
• At least one year of recent experience in Home Health (field experience preferred)
• Strong proficiency with Home Care Home Base (HCHB) and OASIS required
• Independent, organized, and motivated work style
• Reliable transportation, valid drivers license, and auto insurance
• Excellent communication, time management, and critical thinking skills
• BSN and prior experience handling a caseload of 2532 points per week preferred
• Solid understanding of Medicare and home health compliance is a plus
