An opportunity is available for an experienced, compassionate Registered Nurse to deliver top-quality home health nursing care. This contract assignment focuses on providing expert, patient-centered care within clients homes, ensuring continuity and excellence throughout the process. Ideal candidates have strong clinical acumen, are comfortable working autonomously, and maintain clear communication with both patients and interdisciplinary care teams.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Conduct comprehensive OASIS assessments, developing and updating individualized care plans.
- Deliver skilled nursing interventions such as wound care, medication administration, IV therapy, and chronic disease management.
- Monitor and evaluate patient health, promptly reporting changes to healthcare providers.
- Educate patients and caregivers on disease management, medications, and preventive care strategies.
- Document all care accurately and efficiently within Home Care Home Base (HCHB).
- Coordinate patient care with therapists, social workers, and home health aides.
- Attend case conferences, participate in staff meetings, and engage in ongoing professional education.
- Comply with all agency guidelines and relevant regulations for documentation and care delivery.
Qualifications:
- Active, unencumbered RN license in the state of practice.
- Minimum 1 year of recent field experience as a Home Health RN.
- Proficiency in Home Care Home Base (HCHB) and experience with OASIS assessments.
- Reliable transportation, valid drivers license, and auto insurance.
- Strong clinical assessment skills, with the ability to work independently.
- BSN preferred; prior experience managing a caseload of 2532 points per week and knowledge of Medicare/home health compliance is a plus.
