Delivering professional, patient-focused nursing care in the home setting, this contract opportunity is perfect for RNs skilled in Home Health. As the primary clinical resource for your patients, you'll conduct comprehensive assessments, develop evidence-based care plans, and collaborate with a multidisciplinary team to ensure seamless, quality care.

Schedule and Caseload:

• Full-time contract (13 weeks)
• Monday Friday, 8:00 AM 5:00 PM
• Field-based in patient homes
• Caseload: 2532 points per week (typically 35 patient visits per day)
• Daily travel within assigned territory (mileage reimbursement provided)

Responsibilities:

• Complete thorough OASIS assessments and maintain timely, accurate documentation within Home Care Home Base (HCHB)
• Deliver hands-on skilled nursing: wound care, medication administration, IV therapy, chronic disease management
• Monitor patient progress, responding to changes and communicating promptly with physicians and care teams
• Educate patients and families on diagnoses, care plans, preventive strategies, and medication administration
• Work collaboratively with therapists, social workers, and home health aides to ensure effective care coordination
• Adhere to regulatory standards and participate in case conferences and team meetings

Desired Experience and Qualifications:

• Current, active RN license in good standing
• At least one year of recent Home Health experience (field work a plus)
• Proficient with Home Care Home Base (HCHB) and OASIS documentation
• Reliable transportation, a valid drivers license, and current auto insurance
• Strong clinical assessment skills, organization, and the ability to work independently
• BSN preferred and familiarity with Medicare/home health compliance standards desirable
