An experienced and compassionate Registered Nurse is needed for a full-time, 13-week contract providing home health services. This role is ideal for an organized, self-motivated clinician seeking the autonomy of field-based practice combined with the structure and support of a dedicated healthcare team.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct thorough OASIS assessments and develop individualized patient care plans.
• Deliver skilled nursing interventions, including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient status, recognize and communicate changes promptly to the healthcare team and physicians.
• Educate patients and families regarding conditions, medication regimens, and preventive strategies.
• Maintain timely, accurate electronic records using Home Care Home Base (HCHB).
• Coordinate multidisciplinary care with therapists, medical social workers, and aides.
• Attend case conferences, ongoing educational sessions, and staff meetings as required.
• Uphold agency and regulatory standards for quality care and documentation.

Qualifications:

• Current, valid RN license in the state of practice.
• At least one year of recent home health nursing experience (field experience preferred).
• Proficient in Home Care Home Base (HCHB) and OASIS assessment tools.
• Excellent critical thinking, assessment, and communication abilities.
• Reliable transportation, drivers license, and proof of auto insurance.
• Ability to independently manage time and schedule in a field-based environment.
• BSN and experience handling a 2532 point weekly caseload preferred.
• Familiarity with Medicare and home health compliance is a plus.
