Step into a rewarding role as a Home Health Registered Nurse, where your clinical expertise and passion for patient-centered care make a meaningful impact every day. In this field-based position, youll deliver skilled nursing services directly in patients homes, ensuring a high standard of independent, holistic care, and consistent communication with patients, families, and care teams.

Schedule:

• Monday to Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract

Key Responsibilities:

• Conduct thorough OASIS assessments and develop individualized care plans for each patient.
• Provide skilled nursing care including wound management, medication administration, IV therapy, and chronic disease monitoring.
• Monitor patient progress, identify changes in condition, and communicate updates promptly to care teams and physicians.
• Educate patients and their families on diagnoses, treatments, and self-care strategies to empower better health at home.
• Complete timely, accurate documentation in Home Care Home Base (HCHB).
• Collaborate seamlessly with therapists, social workers, and home health aides for effective, integrated care.
• Participate in case conferences and professional development as required.
• Maintain safe, regulatory-compliant care practices and documentation standards.
• Daily travel within your assigned territory, with mileage reimbursement provided.

Qualifications:

• Active RN license (in good standing)
• Minimum 1 year of recent Home Health RN experience; field work strongly preferred
• Proficiency in Home Care Home Base (HCHB) and OASIS required
• Strong assessment, critical thinking, and organizational skills
• Reliable transportation, valid drivers license, and auto insurance
• Ability to work independently and manage a variable field schedule
• Excellent communication skills

Preferred:

• BSN degree
• Previous management of a 2532 point caseload
• Understanding of Medicare/home health compliance
