Deliver skilled nursing care and comprehensive assessments to patients in the comfort of their homes as an experienced Home Health Registered Nurse. This contract position offers the opportunity to make a meaningful impact on patient outcomes by ensuring quality, continuity, and compassionate care throughout the home health journey.

Schedule and Details:

• Monday through Friday, 8:00 AM 5:00 PM
• Full-time 13-week contract
• Caseload: 2532 points per week (typically 35 patient visits daily)
• Field-based: travel within an assigned territory (mileage reimbursement provided)
• Documentation: Home Care Home Base (HCHB)
• Assessment: OASIS

Primary Responsibilities:

• Conduct comprehensive OASIS assessments and establish individualized care plans.
• Deliver skilled nursing interventions including wound care, medication management, IV therapy, and chronic disease management.
• Monitor patient progress, identify and communicate changes, and coordinate with the care team and physicians.
• Educate patients and families regarding conditions, treatments, medications, and preventive care.
• Keep timely, accurate documentation in HCHB.
• Collaborate with physical therapists, occupational therapists, and other healthcare professionals to ensure synchronized care.
• Participate in case conferences and continuing education; adhere to regulatory and agency standards.

Required Qualifications:

• Active RN license in good standing within the state.
• Minimum 1 year of recent home health field experience.
• Proficiency with Home Care Home Base and familiarity with OASIS assessments.
• Strong clinical, critical thinking, and independent time management skills.
• Reliable transportation, valid drivers license, and current auto insurance.

Preferred:

• BSN
• Experience managing a 2532 point caseload.
• Knowledge of Medicare and compliance in the home health setting.

$2,011 - $2,127/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
