Qualifications:

• Active, unencumbered RN license
• Demonstrated experience providing home health nursing care
• Strong clinical assessment and critical thinking skills
• Proficient charting and documentation abilities, including electronic health records
• Ability to work independently, managing caseload and patient visits
• Current CPR/BLS certification
• Valid drivers license and reliable transportation for home visits

Schedule/Hours:

• Flexible scheduling based on patient needs and visit assignments

Key Responsibilities:

• Conduct comprehensive assessments and develop personalized care plans for patients in home settings
• Administer medications, treatments, and wound care in accordance with physician orders
• Provide patient and family education on disease process, medication management, and preventive care
• Collaborate with interdisciplinary teams to coordinate patient care and progress
• Accurately complete required documentation and ensure compliance with established protocols and standards
• Respond promptly to changes in patient condition and communicate effectively with physicians and support staff
