A rewarding opportunity is available for a dedicated Home Health Registered Nurse to deliver compassionate, high-quality care to patients in their homes. This full-time, 13-week contract role requires clinical expertise, initiative, and the ability to work independently while maintaining clear communication and documentation standards.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Primary Responsibilities:

• Complete comprehensive OASIS assessments and tailor individualized care plans for each patient.
• Administer skilled nursing services such as wound care, IV therapy, medication management, and chronic disease support.
• Monitor each patients progress, promptly identifying changes in condition and communicating with the broader healthcare team and physicians.
• Educate patients and families on health conditions, treatment protocols, medications, and evidence-based preventative care.
• Ensure timely, accurate documentation using Home Care Home Base (HCHB).
• Collaborate effectively with therapists, social workers, and home health aides for integrative care delivery.
• Participate in case conferences, staff meetings, and professional development activities as required.
• Adhere closely to regulatory requirements and agency standards in both documentation and clinical care.

Qualifications:

• Active RN license in good standing (state of practice).
• At least 1 year of recent home health field experience.
• Hands-on proficiency with Home Care Home Base (HCHB) and OASIS assessments.
• Robust clinical assessment and critical thinking ability.
• Reliable personal transportation, valid drivers license, and proof of auto insurance.
• Superior time management, organizational, and communication skills.
• Bachelor of Science in Nursing (BSN) preferred.
• Prior experience managing 2532 points per week and knowledge of Medicare/home health compliance is an asset.
