Job Description
Deliver compassionate clinical care and support to individuals and families during lifes most sensitive times as a skilled hospice registered nurse. This contract role involves providing expert end-of-life care for patients in their homes or residential settings, ensuring comfort and dignity throughout the hospice journey.
Schedule:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Occasional weekend or on-call rotations as needed
Key Qualifications:
- Active RN license (state of practice, in good standing)
- Minimum of 1 year recent hospice experience
- Proficiency with Home Care Home Base (HCHB) electronic documentation
- Excellent clinical assessment and communication skills
- Sound knowledge of pain management and end-of-life care
- Valid drivers license, reliable transportation, and auto insurance
- Ability to work independently across a field-based schedule
- Compassionate and emotionally resilient approach
- BSN and prior palliative or end-of-life care experience preferred
- Understanding of Medicare hospice regulations and standards is a plus
Key Responsibilities:
- Manage individualized plans for 45 patient visits per day within the assigned territory
- Conduct comprehensive nursing assessments and symptom management
- Administer medications, treatments, and provide interventions per physician orders
- Collaborate with interdisciplinary team (social workers, chaplains, aides, volunteers) to address patient and family needs
- Provide education to patients, caregivers, and families regarding care, comfort, and emotional support
- Complete timely, accurate documentation using HCHB
- Participate in case conferences, team meetings, and ongoing quality initiatives
- Ensure compliance with hospice standards and regulatory requirements
