Job Description

Deliver compassionate clinical care and support to individuals and families during lifes most sensitive times as a skilled hospice registered nurse. This contract role involves providing expert end-of-life care for patients in their homes or residential settings, ensuring comfort and dignity throughout the hospice journey.

Schedule:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Occasional weekend or on-call rotations as needed

Key Qualifications:

• Active RN license (state of practice, in good standing)
• Minimum of 1 year recent hospice experience
• Proficiency with Home Care Home Base (HCHB) electronic documentation
• Excellent clinical assessment and communication skills
• Sound knowledge of pain management and end-of-life care
• Valid drivers license, reliable transportation, and auto insurance
• Ability to work independently across a field-based schedule
• Compassionate and emotionally resilient approach
• BSN and prior palliative or end-of-life care experience preferred
• Understanding of Medicare hospice regulations and standards is a plus

Key Responsibilities:

• Manage individualized plans for 45 patient visits per day within the assigned territory
• Conduct comprehensive nursing assessments and symptom management
• Administer medications, treatments, and provide interventions per physician orders
• Collaborate with interdisciplinary team (social workers, chaplains, aides, volunteers) to address patient and family needs
• Provide education to patients, caregivers, and families regarding care, comfort, and emotional support
• Complete timely, accurate documentation using HCHB
• Participate in case conferences, team meetings, and ongoing quality initiatives
• Ensure compliance with hospice standards and regulatory requirements
