An exciting contract opportunity exists for an experienced and compassionate Home Health Registered Nurse (RN) dedicated to delivering high-quality patient care in a field-based setting. This role is vital in empowering patients and families in their own homes, ensuring the best possible health outcomes and supporting continuity of care.

Schedule:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Daily travel to patient homes within the assigned territory (mileage reimbursement provided)
• Caseload expectation: 2532 points per week (typically 35 patient visits daily)

Key Responsibilities:

• Complete comprehensive OASIS assessments and develop individualized care plans to address patient needs.
• Deliver skilled nursing interventions including wound care, IV therapy, medication administration, and chronic disease management.
• Monitor and evaluate patient conditions, reporting changes to physicians and multidisciplinary teams promptly.
• Teach patients and family members about disease processes, treatment protocols, medication administration, and preventive care.
• Document care accurately and efficiently using Home Care Home Base (HCHB).
• Collaborate with therapists, social workers, and home health aides to deliver holistic patient care.
• Participate in case conferences and ongoing professional development.
• Ensure compliance with regulatory standards and quality care guidelines.

Qualifications:

• Active RN license in the state of practice, in good standing.
• Minimum one year of recent field Home Health RN experience.
• Demonstrated proficiency with Home Care Home Base (HCHB) and OASIS assessments.
• Reliable transportation, valid drivers license, and auto insurance.
• Excellent organizational, time management, and communication skills.
• Flexibility to work independently and effectively manage a field caseload.
• BSN and experience with Medicare home health compliance standards preferred.

$1,897 - $2,007/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
