Deliver compassionate hospice care and make a meaningful impact in the lives of patients and their families. This contract opportunity calls for an experienced Registered Nurse with a foundation in home health and hospice to manage end-of-life care within residential settings, ensuring dignity and comfort for every patient.

Schedule/Hours:

• Monday Friday, 8:00 AM 5:00 PM
• Occasional weekend or on-call rotation may be required

Key Responsibilities:

• Conduct comprehensive assessments and develop individualized hospice care plans for each patient
• Deliver skilled nursing care focused on symptom management, comfort, and pain control
• Administer medications and treatments in line with physician directives and established hospice protocols
• Closely monitor changes in patient condition, promptly communicating updates to the care team and hospice medical director
• Educate patients, families, and caregivers on disease progression, comfort measures, and emotional support techniques
• Collaborate with interdisciplinary team members, including social workers, chaplains, aides, and volunteers, to address holistic patient needs
• Accurately and efficiently document care using Home Care Home Base (HCHB)
• Participate in case conferences, team meetings, and quality improvement initiatives
• Ensure compliance with all relevant hospice policies, procedures, and regulatory standards

Required Qualifications:

• Active RN license in good standing for the state of practice
• At least 1 year of recent hospice nursing experience
• Proficient with Home Care Home Base (HCHB) electronic documentation
• Strong clinical assessment and communication skills
• In-depth understanding of pain management, end-of-life care, and family support
• Reliable transportation, valid drivers license, and auto insurance
• Compassion, resilience, and the ability to work independently in the field

Preferred Qualifications:

• BSN degree
• Experience in palliative care
• Familiarity with Medicare hospice regulations and documentation standards
