Youll play a critical role ensuring patient well-being, facilitating recovery, and promoting autonomy outside clinical settings while working closely with multidisciplinary teams.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Complete comprehensive OASIS assessments and develop patient-centered care plans
• Deliver skilled nursing interventions such as wound care, medication administration, IV therapy, and chronic disease management
• Closely monitor patients conditions and communicate changes promptly to the care team and physicians
• Educate patients and families on conditions, treatment, medications, and wellness strategies
• Accurately document all care and visits using the Home Care Home Base (HCHB) system
• Collaborate with physical therapists, occupational therapists, social workers, and aides to coordinate care
• Participate in case conferences, meetings, and educational opportunities as appropriate
• Maintain compliance with all regulatory and agency standards for home health services

Qualifications:

• Active RN license in good standing for the state of practice
• At least one year of recent home health field experience
• Proficiency with Home Care Home Base (HCHB) documentation system
• OASIS assessment experience required
• Strong clinical assessment, time management, and independent decision-making skills
• Reliable transportation, valid drivers license, and auto insurance necessary
• BSN and experience handling 2532 points per week preferred
• Familiarity with Medicare/home health compliance a plus

$1,954 - $2,067/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
