Experienced Home Health Registered Nurses are needed for a full-time contract opportunity providing skilled nursing care to patients in their homes. This field-based position plays an essential role in supporting patient independence, safety, and recovery while collaborating with a multidisciplinary healthcare team. The successful candidate will demonstrate strong clinical skills, compassion, and expertise in home health settings.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Perform OASIS assessments and establish individualized care plans.
• Deliver a range of skilled nursing servicessuch as wound care, IV therapy, medication administration, and chronic disease management.
• Monitor patient conditions and progress, notifying care team members of changes as appropriate.
• Educate patients and families on diagnoses, medications, and home safety practices.
• Accurately complete electronic documentation using Home Care Home Base (HCHB).
• Work collaboratively with therapists, social workers, and aides to facilitate comprehensive care delivery.
• Attend case conferences, staff meetings, and ongoing educational trainings.
• Ensure strict compliance with agency and industry standards.

Qualifications:

• Valid and active RN license in good standing.
• Minimum of 1 year of recent field home health RN experience.
• Strong proficiency with OASIS and Home Care Home Base (HCHB) systems.
• Reliable vehicle, valid drivers license, and current auto insurance.
• Excellent assessment skills, independence, and attention to detail.
• Superior interpersonal, communication, and organizational abilities.
• BSN preferred; prior experience managing 2532 points per week a plus.
• Familiarity with Medicare and home health regulations highly desirable.

$1,724 - $1,824/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
