We are seeking a dedicated and experienced Registered Nurse to deliver high-quality skilled care to patients in a home health setting. This field-based role is ideal for a compassionate professional who excels at clinical assessments, patient education, and care coordination. Youll play a key role in improving patient outcomes and ensuring comfort where it matters mosttheir home.

Schedule and Contract Details:

• Full-time, 13-week assignment
• Monday Friday, 8:00 AM 5:00 PM
• Daily travel within an assigned territory; mileage reimbursement included
• Caseload: 2532 points per week, typically 35 patient visits per day

Key Responsibilities:

• Conduct OASIS assessments and develop individualized patient care plans
• Provide skilled nursing services including wound care, medication administration, IV therapy, and chronic disease management
• Evaluate patient status, update care plans, and promptly communicate any changes to the care team and physicians
• Educate patients and families on diagnoses, medications, and ongoing care needs
• Maintain accurate and timely electronic documentation (using Home Care Home Base)
• Collaborate effectively with interdisciplinary healthcare providers
• Attend case conferences, staff meetings, and participate in continuing education as needed
• Follow all regulatory and agency standards for documentation and service delivery

Qualifications:

• Current, active RN license (state of practice)
• Minimum 1 year of recent field-based home health experience
• Home Care Home Base and OASIS experience required
• Reliable transportation, valid drivers license, and auto insurance
• Strong assessment, communication, and organizational skills
• Ability to work independently and manage a flexible schedule
• BSN and Medicare home health compliance knowledge preferred

$2,016 - $2,132/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
