Job Description
A skilled and compassionate Home Health Registered Nurse (RN) is sought to deliver expert nursing care in patients homes throughout the Marietta, GA area. This full-time, 13-week contract role offers a dynamic field-based setting where autonomy, clinical excellence, and patient-focused care converge. The position requires managing a caseload averaging 2532 points weekly, usually consisting of 35 patient visits a day, Monday through Friday from 8:00 AM to 5:00 PM.
Key Responsibilities:
- Conduct detailed OASIS assessments and develop personalized care plans.
- Administer skilled nursing care including wound care, medication management, IV therapy, and chronic disease monitoring.
- Continuously evaluate patient progress, promptly communicating changes to care teams and physicians.
- Educate patients and families on health conditions, treatment protocols, medications, and preventive measures.
- Accurately document all care activities and assessments in the Home Care Home Base (HCHB) system.
- Collaborate with therapists, social workers, and home health aides to ensure seamless, coordinated care.
- Participate in case conferences, team meetings, and ongoing education opportunities.
- Comply with all regulatory standards and agency policies regarding patient care and documentation.
Qualifications:
- Current, active RN license in good standing within the state.
- Minimum of one year recent Home Health experience; field experience preferred.
- Proficiency with HCHB electronic documentation and OASIS assessment tools.
- Strong clinical assessment capabilities and critical thinking skills.
- Reliable transportation, valid drivers license, and auto insurance.
- Ability to work independently with excellent organizational and communication skills.
Preferred:
- Bachelor of Science in Nursing (BSN).
- Previous experience managing caseloads of 2532 points weekly.
- Familiarity with Medicare regulations and home health compliance.
