An exciting opportunity awaits an experienced and compassionate Registered Nurse with a background in home health care. This contract position involves delivering skilled nursing services to patients in their residences, ensuring high-quality, patient-centered care tailored to individual needs. The role is best suited for someone who thrives working independently, demonstrates strong clinical judgment, and excels at patient and family communication.
Schedule & Assignment Details:
- 13-week contract, full-time
- Monday Friday, 8:00 AM 5:00 PM
- Field-based; daily travel to patient homes within your assigned territory (mileage reimbursement provided)
- Expected caseload of 2532 points per week (typically 35 patient visits daily)
- Electronic charting via Home Care Home Base (HCHB) and OASIS assessment tool
Key Responsibilities:
- Conduct OASIS assessments and develop individualized care plans for patients
- Deliver a range of skilled nursing care such as wound care, IV therapy, medication administration, and chronic disease management
- Monitor patient progress, communicate status changes to physicians and the care team, and document all care activities in HCHB
- Educate patients and families on managing health conditions and prevention strategies
- Collaborate with physical therapists, occupational therapists, social workers, and home health aides to coordinate care
- Adhere to agency and regulatory standards for care delivery and documentation
Desired Qualifications:
- Current RN license (in good standing)
- Minimum of 1 year recent home health RN experience (field experience preferred)
- Working knowledge of HCHB and OASIS required
- Proficient with clinical assessments and critical thinking
- Reliable transportation, valid drivers license, and auto insurance
- Strong organizational and communication abilities
- BSN and Medicare/home health compliance knowledge preferred
- Experience handling a 2532 point caseload is beneficial
