This contract role calls for an experienced Home Health Registered Nurse (RN) who thrives in a field-based environment, overseeing comprehensive care delivery to patients in their homes.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM

Position highlights include managing a caseload of 2532 points per week (typically 35 patient visits daily) with a focus on continuity, communication, and clinical excellence. Utilize your expertise with electronic charting using Home Care Home Base (HCHB) and your skill in conducting OASIS assessments.

Key responsibilities:

  • Perform thorough OASIS assessments and design individualized care plans for each patient.
  • Deliver skilled nursing services such as wound care, IV therapy, medication administration, and chronic disease management.
  • Provide patient and family education regarding health conditions, medications, and overall care.
  • Monitor patient progress, report changes promptly, and coordinate care with the healthcare team.
  • Ensure all documentation is accurate and updated within HCHB.
  • Collaborate with interdisciplinary professionalsincluding therapists, social workers, and aidesto achieve positive outcomes.
  • Attend team meetings and participate in continuing education.
  • Maintain compliance with regulatory and agency protocols.

Required qualifications:

  • Active RN license, in good standing.
  • At least 1 year of recent home health experience (field-based preferred).
  • Demonstrated proficiency with Home Care Home Base (HCHB) and OASIS assessments.
  • Strong assessment, time management, and organizational skills.
  • Reliable transportation, current drivers license, and auto insurance.

Preferred qualifications:

  • Bachelor of Science in Nursing (BSN).
  • Previous experience with 2532 point caseloads.
  • Understanding of Medicare and home health guidelines.
