Experienced and compassionate Registered Nurses with expertise in home health are needed for a rewarding contract opportunity. In this vital role, you will deliver skilled nursing care to patients in the comfort of their own homes, ensuring high-quality, patient-focused outcomes. Candidates should be comfortable working autonomously, possess strong clinical assessment skills, and excel at communication to coordinate care and educate patients and their families.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Responsibilities:
- Perform comprehensive OASIS assessments to develop individualized care plans
- Deliver hands-on skilled nursing care, including wound management, IV therapy, medication administration, and chronic illness support
- Monitor patient progress, recognize and report condition changes, and collaborate with physicians and interdisciplinary teams
- Document visits and outcomes using Home Care Home Base (HCHB)
- Educate patients and their families regarding treatment regimens, medications, and disease processes to encourage compliance and understanding
- Coordinate with home health aides, physical therapists, occupational therapists, and medical social workers to ensure optimal care delivery
- Attend case conferences and maintain compliance with regulatory standards and agency policies
Qualifications:
- Active RN license in good standing, issued by the state of practice
- At least 1 year of recent home health nursing experience (field roles strongly preferred)
- Proficiency in Home Care Home Base (HCHB) and OASIS documentation required
- Strong organizational, time management, and communication abilities
- Capability to manage 2532 points per week, typically 35 visits per day
- Must have valid drivers license, reliable transportation, and auto insurance
- BSN and familiarity with Medicare/home health regulations are preferred
