Job Description

An experienced and compassionate Registered Nurse (RN) specializing in home health is needed to deliver skilled nursing care to patients within their homes. This role focuses on providing high-quality, patient-centered care through comprehensive assessments, personalized care plans, and collaboration with interdisciplinary teams. This role is based in Manchester, NH, offering the opportunity to serve patients in a vibrant and welcoming community. Key Responsibilities:

  • Conduct thorough OASIS assessments and develop individualized care plans.
  • Deliver skilled nursing services, including wound care, medication administration, IV therapy, and chronic disease management.
  • Monitor patient progress, recognize changes in condition, and communicate findings to the care team and physicians promptly.
  • Educate patients and families on diagnosis, treatments, medications, and preventive strategies.
  • Maintain timely and accurate documentation using the Home Care Home Base (HCHB) system.
  • Collaborate closely with therapists, social workers, and home health aides for coordinated patient care.
  • Participate in case conferences, ongoing staff meetings, and continuing education initiatives.
  • Ensure compliance with all regulatory and agency standards regarding patient care and documentation.

Qualifications and Desired Experience:

  • Active RN license in good standing.
  • Minimum 1 year recent home health experience, with field-based experience preferred.
  • Proficiency with Home Care Home Base (HCHB) and OASIS assessment tools required.
  • Strong clinical judgment and critical thinking skills.
  • Reliable transportation, valid drivers license, and insurance are needed for daily travel to patient homes.
  • Excellent organizational, communication, and time management abilities.
  • BSN preferred.
  • Experience managing a caseload of 2532 points weekly is a plus.
  • Knowledge of Medicare and home health compliance standards preferred.

Schedule:

  • Full-time, 13-week contract
  • Monday Friday, 8:00 AM 5:00 PM
  • Typically 35 patient visits per day, totaling 2532 points per week
