Job Description

Deliver compassionate, high-quality end-of-life care as a dedicated Hospice Registered Nurse within a collaborative, interdisciplinary home health environment. In this field-based contract role, you will work closely with patients and families, ensuring comfort, dignity, and supportive care throughout the hospice journey.

Schedule

  • Monday to Friday, 8:00 AM 5:00 PM
  • Occasional weekend or on-call rotation may be required

Key Responsibilities

  • Conduct comprehensive nursing assessments and develop individualized hospice care plans
  • Deliver direct skilled nursing care focused on symptom management, pain control, and patient comfort in patients residences
  • Administer medications, treatments, and interventions according to physician orders and hospice protocols
  • Monitor and promptly report changes in patient condition to the interdisciplinary care team
  • Educate patients, families, and caregivers on disease progression, comfort measures, and emotional support
  • Collaborate with social workers, chaplains, and aides to address the full spectrum of patient needs
  • Document care thoroughly using Home Care Home Base (HCHB)
  • Participate in case conferences, team meetings, and quality improvement initiatives
  • Ensure compliance with hospice policies, procedures, and relevant regulations

Qualifications

  • Active RN license (in good standing)
  • At least 1 year of recent hospice experience
  • Proficiency with Home Care Home Base (HCHB) electronic documentation is required
  • Strong clinical assessment, communication, and interpersonal skills
  • Solid understanding of pain management and end-of-life care principles
  • Reliable transportation, valid drivers license, and proof of auto insurance
  • Ability to work autonomously in the field and manage a flexible caseload
  • Compassionate and resilient approach to patient and family care
  • BSN and prior palliative care or hospice setting experience preferred
  • Knowledge of Medicare hospice regulations is a plus
