Job Description
Deliver essential care and comfort as a Hospice Registered Nurse (RN) supporting patients and families through their end-of-life journey. This contract role calls for skilled nursing professionals who value collaboration, compassion, and the highest standards in hospice care, serving individuals in their homes or residential communities.
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM
- Occasional weekend or on-call responsibilities as needed
- Approximately 45 patient visits per day
Key Responsibilities:
- Conduct thorough nursing assessments and develop individualized hospice care plans.
- Deliver skilled nursing interventions, focusing on symptom management, pain control, and patient comfort.
- Administer medications and treatments per physician orders and hospice protocols.
- Closely monitor patient conditions and promptly update the care team and hospice medical director.
- Educate patients, families, and caregivers on disease progression, comfort measures, and emotional support strategies.
- Collaborate with interdisciplinary team membersincluding social workers, chaplains, and aidesto meet physical, emotional, and spiritual needs.
- Accurately complete documentation in Home Care Home Base (HCHB).
- Participate in case conferences, team meetings, and quality improvement efforts.
- Ensure adherence to hospice regulatory guidelines and best practices.
Qualifications:
- Active RN license in good standing
- At least one year of recent hospice experience
- Proficiency with Home Care Home Base (HCHB) documentation
- Strong assessment, communication, and organizational skills
- Reliable transportation, valid drivers license, and car insurance
- Independent and adaptable work style combined with emotional resilience
Preferred:
- Bachelor of Science in Nursing (BSN)
- Background in palliative or end-of-life care
- Familiarity with Medicare hospice requirements
