Job Description
An experienced and compassionate Home Health Registered Nurse (RN) is needed to deliver skilled nursing care to patients in their homes, emphasizing quality and patient-centered care. This field-based role demands self-motivation, strong organizational skills, and the ability to work independently while upholding clinical excellence and clear communication with patients and healthcare teams.
Responsibilities include conducting comprehensive OASIS assessments, developing individualized care plans, administering wound care, medications, IV therapy, and managing chronic diseases. The RN will monitor patient progress, document accurately using Home Care Home Base (HCHB), and collaborate with interdisciplinary professionals such as therapists and social workers to ensure seamless care coordination. Participation in case conferences and ongoing education will support compliance with regulatory standards and enhance care delivery.
Qualifications and Desired Experience:
- Active RN license in good standing for the state of practice
- Minimum of 1 year recent home health experience, preferably in the field
- Proficiency with Home Care Home Base (HCHB) and OASIS tools
- Strong clinical assessment, critical thinking, and communication skills
- Reliable transportation, valid drivers license, and insurance
- Ability to work independently with effective time management
- BSN preferred
- Experience managing 2532 points per week is a plus
- Knowledge of Medicare and home health compliance standards preferred

Job Details:
- Full-time 13-week contract
- Schedule: Monday Friday, 8:00 AM 5:00 PM
- Caseload: 2532 points weekly (approximately 35 patient visits per day)
- Daily travel to patient homes within assigned territory, with mileage reimbursement
