Job Description
Deliver compassionate, high-quality end-of-life care as a Hospice Registered Nurse (RN), supporting patients and families in home and residential settings. This vital contract role offers the opportunity to make a meaningful difference by providing personalized care and comfort during one of lifes most significant journeys. Youll work with an interdisciplinary teamincluding social workers, chaplains, and aidesto ensure a holistic, patient-centered experience.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional weekend or on-call rotation may be required
Key Responsibilities:
- Complete thorough, individualized nursing assessments and develop comprehensive hospice care plans
- Deliver skilled nursing care emphasizing symptom management, pain control, and patient comfort
- Administer medications and treatments in accordance with physician orders and hospice guidelines
- Monitor patient conditions, promptly communicating updates to the care team and hospice medical director
- Educate patients, families, and caregivers on disease progression, comfort measures, and emotional support
- Collaborate with interdisciplinary team members to address the full spectrum of patient and family needs
- Maintain accurate, timely documentation using Home Care Home Base (HCHB)
- Participate in case conferences, team meetings, and quality improvement initiatives
- Ensure compliance with hospice policies, procedures, and regulatory guidelines
Qualifications:
- Active RN license (in good standing)
- At least 1 year of recent hospice experience
- Proficiency in Home Care Home Base (HCHB)
- Excellent clinical, assessment, and communication skills
- Strong understanding of pain management and family support in end-of-life care
- Reliable transportation, valid drivers license, and auto insurance
- Ability to work independently and manage a field-based schedule
- Compassionate, emotionally resilient demeanor
- (Preferred) BSN and/or palliative/end-of-life care experience; knowledge of Medicare hospice documentation
