Providing compassionate end-of-life care requires skill, empathy, and dedication. This full-time Hospice Registered Nurse (RN) role offers the chance to deliver exceptional hospice care to patients in home and residential settings within the Madera, CA area. As a field-based Hospice RN, you will play a vital role in managing symptom control, enhancing patient comfort, and supporting families through this meaningful journey.
Key Responsibilities:
- Perform comprehensive nursing assessments and develop individualized hospice care plans
- Deliver skilled nursing care focused on pain management and symptom relief
- Administer medications and treatments according to physician orders and hospice protocols
- Monitor patient condition changes and communicate promptly with care teams and medical directors
- Educate patients, caregivers, and families on disease progression and comfort strategies
- Collaborate with interdisciplinary team members including social workers, chaplains, and aides
- Complete accurate documentation using Home Care Home Base (HCHB)
- Participate in team meetings, case conferences, and quality improvement efforts
- Ensure adherence to hospice policies and regulatory guidelines
Qualifications & Desired Experience:
- Active RN license in good standing within the state of California
- Minimum of 1 year hospice nursing experience
- Proficient in Home Care Home Base (HCHB) documentation system
- Strong clinical assessment and communication skills
- Adept in pain management and family support principles
- Valid drivers license, reliable transportation, and auto insurance
- Ability to work independently and manage a patient-centered field schedule
- BSN preferred; experience with Medicare hospice regulations is a plus
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional weekend or on-call rotation may be required
