Schedule:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Caseload of 2532 points per week (typically 35 patient visits daily)

Core Responsibilities:

• Perform comprehensive OASIS assessments and develop tailored patient care plans
• Deliver skilled nursingwound care, medication administration, IV therapies, and management of chronic illnesses
• Monitor patient progress, recognize changes in condition, and coordinate with physicians and interdisciplinary team
• Educate patients and families on care routines, medications, health conditions, and preventive practices
• Accurately document all care and assessments using Home Care Home Base (HCHB)
• Participate in team meetings, case conferences, and ongoing professional development
• Drive daily to patient homes within assigned areas (mileage reimbursement provided)

Required Qualifications:

• Active RN license in good standing
• Minimum 1 year of recent Home Health nursing experience (field experience strongly preferred)
• Proficiency with Home Care Home Base (HCHB) and OASIS assessments
• Strong assessment, organization, and critical thinking abilities
• Reliable transportation, valid drivers license, and current auto insurance
• Comfortable working independently and coordinating field-based care schedules

Preferred Qualifications:

• BSN degree
• Experience managing a caseload of 2532 points per week
• Understanding of Medicare and home health compliance regulations

$2,069 - $2,189/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
