Job Description

Deliver skilled nursing care in patients homes as a Home Health Registered Nurse (RN), providing compassionate, patient-focused services that promote healing and independence. This full-time contract opportunity involves working independently in a field-based setting while maintaining high clinical standards and effective communication with patients, families, and healthcare teams.

Location:
Based in Lynchburg, VA, this position offers the chance to work within the local community, providing vital home health care to patients in a variety of residential settings. Key Responsibilities:

  • Conduct thorough OASIS assessments and develop individualized care plans tailored to patient needs.
  • Deliver skilled nursing interventions such as wound care, medication administration, IV therapy, and chronic disease management.
  • Monitor patient progress, recognize changes in condition, and communicate effectively with physicians and interdisciplinary teams.
  • Educate patients and families on disease management, treatments, medications, and preventive care.
  • Document care accurately and timely using the Home Care Home Base (HCHB) electronic system.
  • Collaborate with therapists, social workers, and home health aides to ensure coordinated, comprehensive care.
  • Participate in case conferences, staff meetings, and continuous learning efforts.
  • Ensure all care practices comply with regulatory and agency standards.

Qualifications & Desired Experience:

  • Active RN license in good standing for the state of practice.
  • Minimum of 1 year recent home health experience; field experience preferred.
  • Proficient with Home Care Home Base (HCHB) and proficient in OASIS assessments.
  • Strong clinical assessment, critical thinking, and patient communication skills.
  • Reliable transportation with valid driver's license and auto insurance for daily travel.
  • Ability to manage a flexible, independent field-based schedule.
  • BSN degree preferred.
  • Familiarity with Medicare and home health compliance standards is a plus.
  • Previous management of 2532 points per week caseload preferred.

Schedule:

  • Monday through Friday, 8:00 AM 5:00 PM
  • Expect 35 patient visits per day, averaging 2532 points weekly.
