Providing skilled nursing care to patients in their homes, this role offers a rewarding opportunity to deliver patient-centered care within a flexible, field-based setting. As a Home Health Registered Nurse, you will perform in-depth assessments, develop personalized care plans, and coordinate with interdisciplinary teams to ensure optimal patient outcomes. This full-time, 13-week contract position involves daily travel within the assigned territory, supported by mileage reimbursement.

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans.
• Deliver skilled nursing care including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, recognize changes in condition, and communicate effectively with physicians and the care team.
• Educate patients and families on disease processes, treatments, medications, and preventive care.
• Maintain accurate documentation using Home Care Home Base (HCHB).
• Collaborate with physical therapists, occupational therapists, medical social workers, and home health aides.
• Participate in case conferences, staff meetings, and continuing education.
• Ensure compliance with regulatory standards and agency protocols.

Qualifications and Desired Experience:

• Active RN license in the state of practice (in good standing).
• Minimum of 1 year recent home health experience with field work preferred.
• Proficiency with Home Care Home Base (HCHB) and experience with OASIS.
• Strong clinical assessment, critical thinking, time management, and communication skills.
• Reliable transportation, valid drivers license, and auto insurance.
• Ability to work independently and manage a flexible field schedule.
• BSN preferred.
• Familiarity with Medicare and home health compliance standards.
• Experience managing 2532 points per week is advantageous.

Schedule:

• Monday to Friday
• 8:00 AM 5:00 PM
• Typical caseload of 2532 points weekly (35 patient visits per day)
