Job Description
Deliver skilled nursing care in patients homes as a Home Health Registered Nurse (RN), focusing on quality, continuity, and patient-centered treatment. This full-time, 13-week contract position offers a Monday through Friday schedule from 8:00 AM to 5:00 PM, involving field-based patient visits within an assigned territory near Lowell, MA. You will perform thorough assessments, develop personalized care plans, and collaborate with interdisciplinary teams to ensure optimal patient outcomes.
Key Responsibilities:
- Conduct comprehensive OASIS assessments and develop individualized care plans.
- Provide skilled nursing services such as wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient progress, recognize changes in condition, and communicate promptly with physicians and care teams.
- Educate patients and families regarding disease management, treatments, medications, and preventive care.
- Maintain accurate documentation using Home Care Home Base (HCHB) electronic system.
- Coordinate care with physical therapists, occupational therapists, medical social workers, and home health aides.
- Participate in case conferences, team meetings, and continuing education activities.
- Ensure compliance with regulatory and agency standards for care and documentation.
Qualifications:
- Active RN license in good standing within the state of practice.
- Minimum of 1 year recent home health experience, preferably field-based.
- Proficient with Home Care Home Base (HCHB) and experienced with OASIS assessments.
- Strong clinical assessment and critical thinking abilities.
- Valid drivers license, reliable transportation, and auto insurance.
- Ability to work independently and manage a flexible, patient-home visit schedule.
- Excellent organizational, communication, and time management skills.
Preferred:
- Bachelor of Science in Nursing (BSN).
- Experience managing 2532 points per week caseload.
- Knowledge of Medicare and home health regulatory compliance.
