An exciting opportunity is available for an experienced and compassionate Registered Nurse specializing in home health care. This contract position requires a motivated and organized nurse who thrives in an independent, field-based setting, delivering patient-centered, high-quality care directly in patients homes.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct comprehensive assessments using OASIS and develop individualized care plans.
• Provide skilled nursing interventions, including wound care, IV therapy, medication administration, and chronic disease management.
• Monitor patient progress, promptly communicate changes to the care team and physicians, and adjust treatments as needed.
• Educate patients and families about disease processes, medications, and preventive strategies to maximize recovery and independence.
• Complete all documentation accurately and on schedule in Home Care Home Base (HCHB).
• Collaborate with therapists, medical social workers, and home health aides to deliver coordinated, multidisciplinary care.
• Participate in case conferences, staff meetings, and ongoing continuing education.
• Ensure compliance with all organizational and regulatory documentation standards.

Required Qualifications:

• Active, unencumbered RN license in the state of practice.
• Minimum of one year of recent home health nursing experience (field experience strongly preferred).
• Proficiency with Home Care Home Base (HCHB) and OASIS documentation.
• Excellent clinical assessment, critical thinking, and communication skills.
• Able to manage a flexible, field-based caseload (2532 points per week; about 35 home visits daily).
• Reliable vehicle, current drivers license, and valid auto insurance.
• Strong organizational and time-management skills.

Preferred Qualifications:

• BSN degree.
• Prior experience managing a home health caseload of 2532 points per week.
• Understanding of Medicare and home health compliance standards.
