Job Description

Deliver compassionate, high-quality care as a Home Health Registered Nurse, providing skilled nursing services directly in patient homes. Your clinical judgment and ability to work autonomously will be essential as you support patients health goals, collaborate with interdisciplinary providers, and educate families to ensure outstanding outcomes.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

  • Perform thorough OASIS assessments and develop personalized care plans based on patient needs.
  • Deliver skilled nursing care, such as wound care, medication administration, IV therapy, and chronic disease management.
  • Monitor patient condition, recognize changes, and communicate effectively with physicians and care team members.
  • Educate patients and families on diagnoses, medications, treatment regimens, and preventive measures.
  • Ensure meticulous and timely documentation using Home Care Home Base (HCHB).
  • Collaborate with therapists, social workers, and home health aides for integrated patient care.
  • Participate in case conferences, staff meetings, and professional development activities.
  • Adhere to compliance and agency standards for patient care and documentation.
  • Provide daily travel to patient residences within assigned territory (mileage reimbursement provided).

Qualifications:

  • Active RN license (in good standing) in the state of practice.
  • Minimum of one year of recent home health RN experience (field preferred).
  • Demonstrated proficiency with Home Care Home Base (HCHB) and OASIS.
  • Strong clinical assessment, communication, and organizational skills.
  • Reliable transportation, valid drivers license, and proof of auto insurance.
  • Ability to independently manage a field-based schedule.

Preferred:

  • BSN degree.
  • Experience with caseloads of 2532 points per week.
  • Familiar with Medicare and home health compliance standards.
