Deliver compassionate, patient-centered care as an experienced RN specializing in hospice and home health. This contract opportunity involves supporting individuals and families through the end-of-life journey while providing high-quality clinical support and comfort in home or residential settings.

Schedule:

• Monday to Friday, 8:00 AM 5:00 PM
• Occasional weekend or on-call rotation may be required

Key Responsibilities:

• Perform comprehensive nursing assessments; develop and adapt individualized care plans for hospice patients.
• Deliver skilled nursing care, managing symptoms and pain to maximize patient comfort.
• Administer medications and treatments per physician orders and hospice standards.
• Monitor patient status, promptly communicating changes to the care team and medical director.
• Offer education and guidance to patients, families, and caregivers on end-of-life care, medication routines, and support techniques.
• Collaborate with a multidisciplinary teamincluding social workers, aides, chaplains, and volunteersto ensure holistic care.
• Maintain timely and accurate documentation using Home Care Home Base (HCHB).
• Participate in team meetings, case conferences, and continuous quality initiatives.
• Ensure full compliance with hospice protocols and regulatory guidelines.

Qualifications:

• Active RN license (state-specific, in good standing)
• At least one year of recent hospice experience required
• Proficiency in Home Care Home Base (HCHB) required
• Excellent assessment and communication skills
• Strong foundation in pain management and end-of-life principles
• Reliable transportation with valid drivers license and auto insurance
• Demonstrates compassion and emotional resilience
• Ability to work independently in a field-based setting
• BSN and palliative care experience preferred
• Knowledge of Medicare hospice regulation is a plus
