Job Description
Deliver exceptional end-of-life nursing care and support to patients and families as a Hospice RN in home and residential settings. This full-time, 13-week contract role offers the chance to make a meaningful impact during one of life's most significant journeys. You'll blend clinical skills, compassion, and excellent communication to ensure each patient experiences comfort, dignity, and high-quality care.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional weekend or on-call rotation may be required
Qualifications:
- Active RN license (in good standing)
- Minimum 1 year recent hospice nursing experience
- Proficiency in Home Care Home Base (HCHB) electronic documentation
- Excellent clinical assessment and communication abilities
- Understanding of pain management, end-of-life care, and family support
- Reliable transportation, valid driver's license, and auto insurance
- Capacity to work independently and manage a field-based schedule
- Compassionate, emotionally resilient demeanor
- BSN and experience in palliative or end-of-life settings preferred
- Familiarity with Medicare hospice regulations and documentation standards a plus
Key Responsibilities:
- Conduct comprehensive patient assessments and develop individualized hospice care plans
- Deliver skilled nursing focused on symptom management, pain control, and holistic comfort
- Administer medications and interventions per physician orders and protocols
- Monitor and report patient condition changes to the care team and medical director
- Provide patient, family, and caregiver education on care strategies and emotional support
- Collaborate with interdisciplinary team members to address comprehensive patient needs
- Ensure timely, accurate documentation in HCHB
- Participate in team conferences and quality improvement initiatives
- Maintain compliance with regulatory and hospice policies
