Job Description

Deliver compassionate, high-quality end-of-life care by joining a respected interdisciplinary team as a Hospice Registered Nurse. In this full-time, field-based role, you will make a significant impact on patients and families, ensuring comfort and dignity in the concluding stages of life. This is a 13-week contract position, ideal for a nurse who is passionate about home health and hospice care.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Occasional weekend or on-call rotation may be required

Key Responsibilities:

  • Complete thorough nursing assessments and develop individualized hospice care plans
  • Deliver skilled nursing visits (45 daily) focused on symptom management, pain control, and comfort measures
  • Administer medications and interventions in compliance with physician orders and hospice protocols
  • Closely monitor patient conditions, promptly updating the care team and medical director
  • Educate and support patients, families, and caregivers regarding disease process, comfort techniques, and medication management
  • Collaborate with social workers, chaplains, aides, and other team members to address holistic needs
  • Accurately document all care in Home Care Home Base (HCHB)
  • Participate in team conferences, meetings, and quality improvement activities
  • Ensure strict adherence to organizational and regulatory hospice standards

Desired Qualifications:

  • Active RN license in good standing
  • At least one year of recent hospice experience
  • Proficiency with Home Care Home Base (HCHB)
  • Strong assessment, communication, and critical thinking skills
  • Solid understanding of pain management and end-of-life principles
  • Reliable transportation, valid drivers license, and auto insurance
  • Compassionate, resilient, and able to work independently in the field
  • BSN preferred; previous palliative care experience beneficial
  • Familiarity with Medicare hospice regulations a plus
