An opportunity is available for a skilled Home Health Registered Nurse (RN) to deliver exceptional nursing care in patients' homes. This contract position involves managing a caseload of 2532 points weekly, typically 35 patient visits per day, in a dynamic field-based setting throughout the assigned territory. The schedule is Monday through Friday, 8:00 AM to 5:00 PM.

Responsibilities:
- Conduct comprehensive OASIS assessments and develop tailored care plans
- Provide skilled nursing care including wound care, medication administration, IV therapy, and chronic disease management
- Monitor patient progress, communicate changes promptly with physicians and care teams
- Educate patients and families on treatment plans, medications, and preventive health
- Ensure accurate, timely documentation using Home Care Home Base (HCHB)
- Collaborate with interdisciplinary healthcare professionals to coordinate patient care
- Participate in case conferences, staff meetings, and ongoing education
- Comply with all regulatory and agency standards for documentation and care delivery
Qualifications / Desired Experience:
- Active RN license in good standing for the state of practice
- Minimum 1 year recent home health nursing experience, with field experience preferred
- Proficiency in Home Care Home Base (HCHB) and OASIS assessments required
- Strong clinical assessment skills and critical thinking
- Reliable transportation, valid drivers license, and insurance
- Ability to manage flexible schedules independently
- Excellent communication, time management, and organizational skills
- BSN preferred
- Prior experience managing 2532 points per week and knowledge of Medicare/home health compliance a plus
